Patient Medical Records

Please Note: Effective Thursday March 19, 2020, the Medical Records office will be closed to patient walk-in services due to visitor restrictions related to COVID-19.

Patients can use Patient Gateway to access their medical records and request additional records. Patients can also fax an authorization form to the Mass General Brigham Release of Information team at 617.726.3661 or mail the form to 121 Innerbelt Road, Somerville, MA 02143-4453.

How to Access Medical Records

Consistent with current law, McLean Hospital policy allows patients, their families, legal guardians, or healthcare agents to request permission to either inspect a hospital record or obtain a copy of the discharge summary or other portion(s) of a record. A request must be accompanied by the consent of the patient or other legally authorized person. Although most requests are granted, if a determination is made that inspection will result in serious harm to the patient, such access will be denied.

Patients can request copies of, or amendments to, their medical records by submitting forms to the Mass General Brigham Medical Records Department. Find more information on the Mass General Brigham website.
